What is Community Support?
Community Support is a service aimed at addressing all of the rehabilitation needs of an individual with mental health and substance abuse issues. The goal of Community Support is to provide the individual consumer with the necessary supports in place to aid them in their own individual road to recovery. All of the major life domains are addressed in our program, which include the mental health, substance abuse, financial, social, educational, vocational, and housing needs of the individual.
Community Support Team is a more intensive service and individuals receiving this service will receive 8 face-to-face contacts per month.
Community Support Individual is a less intensive service and individuals in this program will receive two contacts per month, at least one of them being face-to-face.
