Mecklenburg Open Door, Inc. works in collaboration with local community and government agencies for referrals into our programs.

Permanent Housing
Most of our programs have a waiting list, therefore it is important to ensure the process for obtaining a spot on the list is completed in a timely fashion.

In order to obtain a spot on the waiting list please complete the Preliminary Application for Assistance and email or fax it to:

Your spot on the list will be determined by the date and time the Preliminary Application was received. Once your preliminary information is processed you will be contacted by one of our housing specialists who will provide further details and information.

Transitional Housing
Transitional Housing is primarily accessed through referrals made by the Mecklenburg County Liaison at Broughton State Hospital, Mecklenburg County Jail Diversion Liaison, and the Mecklenburg County Public Defender's Office.

The Transitional Housing Referral Form can be completed and faxed to MOD's Intake and Assessment Manager at 704-525-0949. Once referrals are made for this service the MOD Intake & Assessment Manager will screen the individual for appropriateness based on program criteria and will coordinate services with the identified individual or supportive services in place.

Community Support
Community Support Services are coordinated through MOD's Clinical Supervisor and must be referred through MeckLINK or by an existing provider for recipients receiving Shelter Plus Care, MOD's TransMED Clinic or Transitional Housing Program.
